Inspector Notes

An unannounced monitoring inspection was completed on 06/27/2025 in response to a self-report received by the licensing office on 06/19/2025 relating to a serious injury to children while in care of the center. The inspector reviewed 2 child records and 3 staff records on-site. This inspection included document review, tour of the facility, interviews, and observations.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.
Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 07/18/2025. A POC submitted after this date will not appear on the public website.

When staff are supervising children, they shall always ensure their care, protection, and guidance. During a naptime fire drill on 06/19/2025, two children exited the building with their class without shoes, and walked on a concrete sidewalk resulting in 2nd degree burns to their feet. The outside temperature during the drill was about 90 degrees.
Plan of Correction: Teacher retraining and coaching will be immediate based on observations. Fire drill safety checks will be conducted by Directors before the fire drill starts.
