Inspector Notes
This inspection was conducted by staff using an alternative remote protocol, including phone interview, records review, and a virtual observations. An on-site inspection was not conducted at this time.
An unannounced renewal inspection was conducted at 10:18am. The provider was contacted by phone to initiate the inspection. There were 2 children present = 4 points with the provider. Ratios and supervision were in compliance at the time of inspection. Children were observed playing with toys during the inspection. The home was sufficiently supplied with toys and equipment for children, and appeared clean and organized today. The provider completed annual training requirements.
Information gathered during the inspection determined there were areas of non-compliance with applicable standards or laws reviewed. Questions about this inspection may be directed to pamela.sneed@doe.virginia.gov

1Evidence:
1. The records for Child #10 + #11 did not include information for parents listed in this regulation section: Operating hours, holidays, fees and payments, check-in and out procedures, medication policy, liability insurance, general daily schedule, discipline policy, provisions of the emergency preparedness plan, and termination policy.
2. While these forms were signed by parents, the forms and information were left blank by the provider.
