Inspector Notes

An unannounced monitoring inspection was conducted at 10:20am. Observed 3 children = 6 points with the Provider. An adult household member was also present. Ratios and supervision were in compliance. Children were playing with toys, and listening to a story being read to them. The daycare areas of the home were found to be clean and sufficiently supplied with toys and equipment for the children. The provider has obtained a substantial amount of annual training in 2022. Areas of non-compliance found. Questions about this inspection may be directed to pamela.sneed@doe.virginia.gov

Based on records reviewed, it was determined that 3 of 3 children?s records did not include a physical exam within the first 30-days of enrollment.

Evidence:

1. Child #1 ? There was no physical exam on-file. The child began attending 9/28/21.
2. Child #2 - There was no physical exam on-file. The child began attending 10/18/21.
3. Child #3 - There was no physical exam on-file. The child began attending 8/23/22.
Plan of Correction: The provider said she would request physical exams for all children enrolled.

Based on records reviewed, it was determined that 3 of 3 children?s records did not include required information. Evidence: The records for Child #1, #2, and #3 did not include a signed liability form.
Plan of Correction: The provider said she would complete the form for all children enrolled and have the parents sign accordingly.
