Inspector Notes

An unannounced, on-site monitoring inspection was conducted 8/15/2024 from 10:05am - 10:55am. The inspector reviewed compliance in the areas listed above. There were 3 children present and 1 caregiver. The inspector reviewed 3 children?s records, 1 caregivers record and 3 household members records onsite. This inspection included document review, interviews, observations, and a tour of the facility.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Standard 8VAC20-800-180-A
Based on record review, the provider did not obtain for each adult household member a current report of tuberculosis (TB) screening every two years from the date of the first screening.

Evidence:
Household member #1 had a TB screening that expired 8/12/2024. At the time of the inspection, there was no documentation of an updated TB.
Plan of Correction: The household member has already had the test but we are waiting for the results to come in the mail.
Standard 8VAC20-800-280
Based on observation, potentially poisonous substances, materials and supplies were not
stored in areas inaccessible to children.

Evidence:
The door to the laundry room was observed to be open, with various cleaning and household substances on the floor or low shelving units. These substances include but are not limited to bottles of laundry of detergent, WD-40, windshield washer fluid and other car maintenance fluids.
Plan of Correction: The door to the laundry room was closed and locked during the inspection.
Standard 8VAC20-800-730-D
Based on observation, medication for caregivers and household members was not kept in a locked place using a safe locking method that prevents access by children.

Evidence:
Household prescription and over the counter medications were observed in an open bin on the kitchen counter while two children were making smoothies. The bin did not have a lid or safe locking method, which made the contents accessible to children.
Plan of Correction: The bin of medication was moved during the inspection.
Standard 8VAC20-800-80-A
Based on record review and interview, the provider did not obtain verification of the identity and age of the child from the parent within 7 business days of the child's first day of attendance at the family day home.

Evidence:
Child #1, enrollment date 8/19/2024, did not have documentation of proof of age and identity. The provider stated the parent does not have a birth certificate for the child and is trying to obtain one.
Plan of Correction: I have already spoken with the parent and she is trying get a copy of the birth certificate. I will ask her to get a letter from the hospital.
Standard 8VAC20-800-920
Based on observation, a menu for the current one-week period was not dated or posted in an area accessible to parents

Evidence:
There was a menu on the refrigerator in the upstairs kitchen, but it was not dated and accessible to parents.
Plan of Correction: The menu was moved to the parent board during the inspection. It will be dated moving forward.
