Inspector Notes
An onsite unannounced monitoring inspection was conducted on June 16, 2022. There were 25 children in care ranged in age from six months to five years old at the onset of the inspection. There were six staff providing direct care to children. A walking tour of the facility inside and outside was completed. Children were observed in teacher and self-directed activities including play activities, restroom routines, nap, snack and outside play. Emergency equipment, annual inspections, and required postings were reviewed. Violations were observed, appear on the violation notice and were discussed with the administrator during the exit interview.
Violations
7Evidence:
1 - Child 2 was observed sleeping in a half size, hard sided port-a-crib on top of a small dog?s cushion which was in the crib on top of the mattress.
2 - Staff 3 stated that the dogs bed was purposefully placed in the crib for child 2 to sleep on.
3 - The fence enclosing the outside play space is leaning outward with a broken post and broken pickets exposed. The exposed edges of the broken post and pickets have sharp jagged edges within reach of children playing on the playground.
The fencing will be repaired
Evidence:
1 - There was a package of disinfecting wipes on a table in the classroom where the three and four-year-old children ate snack. The table is a child height, an unlocked location and accessible to children in care.
2 - There was a package of disinfectant wipes in an unlocked cabinet in the room where four and five-year-old children were napping.
Evidence:
1 - There is less than the required footage around and between the four pieces of equipment. Each piece of equipment requires a six foot fall/use zone that does not overlap with another fall/use zone.
2 - There is less than a six-inch depth of resilient surface throughout the playground under, around and into the fall/use zones of the climb and slide equipment on the playground.
Evidence:
1 - There were nine children present in the infant room with 2 staff were three staff are needed.
a. Children present ranged in age from six months through 18months.
2 - Staff 1, 6 and 7 confirm that there were nine children present in the classroom.
Evidence:
Upon arrival child 1 was observed in the hallway doing push-ups. Staff 2 stated that ?he was moving around and disrupting other children so he was doing push-ups.?
Evidence:
There is an infant bottle in frig in the infant room that is not labeled with the child?s name or date. There are two bottle fed infants in care.
Evidence:
The attendance documentation for children in care in the three - four-year-old room was not available for review including the arrival and departure of each child as it occurs.
