Inspector Notes
An unannounced renewal inspection was initiated on September 12, 2023 and concluded on September 27, 2023. There were 22 children ages 18months through four years old with eight staff and administrators. Children and staff records were requested and reviewed digitally. Children were observed in teacher and self-directed activities. A walking tour of the center's facilities were observed inside and outside. Additionally, practice emergency evacuation documentation, emergency postings/evacuation routes, required postings, fire and health inspections, medication, first aid kit and emergency equipment were reviewed. Violations were observed in the areas of medication, equipment and resilient surface. These violations appear on the violation notice and were discussed with provider at the conclusion of the inspection.

4Evidence:
1 - There is less than six inches of resilient surface under around and into the fall/use zone of climb and slide equipment on the playground.
2 - Staff present on the playground confirmed that there was not six inches of resilient surface under around and in the fall/use zone of climb and slide equipment.
Evidence:
The diaper changing pad in the toddler room is torn and therefore not a nonabsorbent surface.
Evidence:
1 - There is a rectal medication in the locked medication location. However, the child has not been present for care since the beginning of the summer session and the medication was not disposed of as required.
2 - The owner confirmed that the medication had not been disposed of as required.
Evidence:
1 - There is a tube of diaper cream in the infant room that has an expiration date of February 2023.
2 - The owner confirmed that that diaper cream has expired.
