During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced, on-site monitoring inspection was initiated and completed on 11/25/2024, as a part of the licensure period, The on-site inspection began at 10:20am and ended at 12:30pm. The inspector reviewed compliance in the areas listed above. There were 16 children present and 9 staff. The inspector reviewed 4 children?s records and 7 staff records on-site. This inspection included documentation, tour of the facility, interviews, and observations.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 12/27/2024.

Standard 8VAC20-780-330-B
Where playground equipment is provided, resilient surfacing shall comply with minimum safety standards when tested in accordance with the procedures described in the American Society for Testing and Materials. Fall zones are defined as the area underneath and surrounding equipment that requires a resilient surface. A fall zone shall encompass sufficient area to include the child's trajectory in the event of a fall while the equipment is in use. The playground had less than six inches of required resilient surface under around in the fall and use zone of climb and slide equipment that was in use by children in care during the inspection.

The specified staff-to-children ratios are required whenever children are in care. 16 months up to 24 months 1:5. From approximately 9:45am until after 10:20am there were six children in care with one staff. The youngest child present was 23 months old. Therefore, a second staff was required.

Standard 8VAC20-780-560-F
Centers shall follow the most recent, age-appropriate nutritional requirements of the Child and Adult Care Food Program of the United States Department of Agriculture (USDA). Children were served 2-3 chicken nuggets = 13 grams of protein pre nugget, French fries and apple sauce served measured approximately 1/8 ? 1/4 cup of each. These measurements do not meet USDA requirements for the ages of children being served.
