Inspector Notes

A subsidy health and safety inspection (SHSI) was conducted in conjunction with the licensing inspection. Subsidy standards that are not covered in licensing standards were reviewed during this SHSI.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five business days from today, which will be the close of business on 8/28/2025. A POC submitted after this date will not appear on the public website.

Standard 8VAC20-790-170-G
The vendor shall maintain, in a way that is accessible to all caregivers, a current written list of all children's allergies, sensitivities, and dietary restrictions. This list shall be dated and kept confidential.

The vendor had an undated allergy list posted in plain sight on the refrigerator.
Plan of Correction: The list was dated and a cover sheet was added during inspection.
Standard 8VAC20-790-190-B-12
The vendor shall obtain a written care plan for each child with a diagnosed food allergy, to include instructions from a physician regarding the food to which the child is allergic and the steps to be taken in the event of a suspected or confirmed allergic reaction.

Child #2, enrolled for three days, has diagnosed food allergies. The vendor did not have a written care plan from the child's physician.
Plan of Correction: The care plan was requested the same day and parents provided it the following day.

Caregivers who work directly with children shall annually complete the department's health and safety update course.

Caregiver #1 completed their annual health and safety update over five months late.
Plan of Correction: The training was completed the same day as the inspection.
