Inspector Notes

This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, documents review, and a virtual tour of the program.

A renewal inspection was completed on 3/17/2022. The provider was contacted by telephone and a virtual inspection was conducted. There were 7 children present in the before school program (7 points), with the provider supervising. The Inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, special care and emergencies, and nutrition. A total of 2 children's records and 3 caregiver/household member records were reviewed.

Information gathered during the inspection determined non-compliance with applicable standards or law and one violation was documented on the violation notice issued to the program.

Based on a review of caregiver records, it was determined the provider did not maintian for each caregiver a current Report of Tuberculosis Screening form, in accordance with the requirements in 22 VAC 40-111-170, every two years from the date of the first screening.
Evidence:
The provider was not able to demonstrate that she has documentation showing that her repeat TB Health Screening is current.
Plan of Correction: I will return to the doctor for another health screening. The documentation will be placed in my file.
