A caregiver shall be physically present on site and provide direct care and supervision of each child at all times. Direct care and supervision of each child includes: Awareness of and responsibility of each child in care, including being near enough to intervene if needed.

On November 14, 2025, at 3:30pm, Child #1 (age: 5 years) was picked up from the pick up area outside of their school and signed out into the care of the family day home by Caregiver #1. Eleven other children were picked up by Caregiver #1 in the same area with the last child being checked in to care at 3:35pm. Caregiver #1 and the other children left the pickup area of the school, visited the school playground for an undetermined amount of time and then started walking towards the family day home before Caregiver #1 realized that Child #1 was not with the other eleven children. Child #1 was found by a crossing guard in the pick-up area outside of their school and Child #1 was brought to the school's office. The parents of Child #1 and Caregiver #1 were called by the school office and informed that Child #1 was left at school. Child #1 was picked up from the school by their parent. Child #1 was missing from Caregiver #1s care for an undetermined amount of time.
Plan of Correction: I am treating this incident as a critical opportunity to strengthen and improve my program. Here are the measures I will be implementing immediately to ensure BAMM is a safer and more reliable environment:
? Double-counting at every checkpoint. I will conduct two counts each time, and I will use my phone's attendance list to verify each child by name.
? A safety lesson for the children. We will review what they should do if they become separated from the group. This real-life situation provides a meaningful teaching opportunity.
? Stricter lining-up routines. I will reinforce clear, consistent procedures for lining up so that head counts are accurate and efficient. Moving children are much harder to count than still ones.
? A buddy system. Each child will be paired with a sibling or another assigned buddy. This will provide an additional layer of accountability and awareness.
? Additional adult supervision on larger outings. When we visit playgrounds or parks with open or limited-visibility areas
