Inspector Notes
A monitoring inspection was initiated on 9/8/2022 and concluded on 9/8/2022. There were 38 children directly supervised by 7 staff members. Children were observed during engaged in group activities and outdoor play time. The physical plant, 3 child records, 5 new staff records, medications, evacuation drills, emergency supplies and policies were inspected. The playground was measured and classrooms were re-measured. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. Violations
7Evidence: The following staff did not have TB screening at the time of employment and coming into contact with the children, all three staff were observed supervising children this date; Staff # 2, date of employment 8/15/2022, staff # 4, date of employment 8/3/2022 and staff #5, date of employment 8/1/2022.
Evidence: The following staff was observed supervising children this date and there was no documentation of orientation training; Staff # 4, date of employment 8/3/2022 and staff #5, date of employment 8/1/2022.
Evidence: There is one child needing emergency medication for food allergies. There is no one employed that has completed the required training in medication administration of the epi pen medication.
Evidence: Child #1 has a prescription epi pen for food allergies, there is no written authorization from the parent or physician to allow the medication to be administered.
Evidence: The diaper ointments in the toddler class were observed accessible to children in a unlocked cabinet under the changing table.
Evidence: Two children's prepared bottles were observed in the refrigerator, not dated.
Evidence: There was no documentation of two references for the following staff;
Staff # 4, date of hire 8/3/2022 and staff #5, date of hire 6/17/2022.
