Inspector Notes
A subsidy health and safety (SHSI) inspection was conducted on-site April 25, 2023 and concluded remotely on May 8, 2023. There were 63 children present with 13 staff supervising. A licensing inspection was also conducted on this date. Both inspections combined cover subsidy health and safety regulations. Only subsidy requirements that are not covered in licensing standards were reviewed during this supplemental inspection.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.
Please complete the ?plan of correction? and ?date to be corrected? for each violation cited on the violation notice and return it to me within 5 business days from today. Please specify how the deficient practice will be or has been corrected. Your plan of correction should contain: 1) steps to correct the noncompliance with the standard(s), 2) measures to prevent the noncompliance from occurring again; and 3) person(s) responsible for implementing each step and/or monitoring any preventative measures. Please do not use staff names; list staff by positions only.

3Evidence: The record of staff #3 (hired 12/03/20) contained documentation of CPR certification that expired March 2021. Staff #3 acknowledged the staff didn't have a current CPR certification.
Evidence: The record of staff #3 (hired 12/03/20) contained documentation of first aid that expired March 2021. Staff #3 acknowledged the staff didn't have a current first aid certification.
Evidence: The record of staff #3 contained documentation of the annual Health and Safety update for 2023. The 2022 annual Health and Safety training was dated 01/22/22.
