During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.
Inspector Notes
An unannounced, on-site renewal inspection was initiated on 4/7/2025 and completed on 4/21/2025, as part of the licensure period. The on-site inspection began at 3:30pm and ended at 4:40pm. The inspector reviewed compliance in the areas listed above. There were 22 children present and 1 staff. The inspector reviewed 2 children?s records on site and staff records electronically on 4/16/2025.
This inspection included document review, tour of the facility, and interviews.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.
Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 4/30/2025. A POC submitted after this date will not appear on the public website.
Violations
6Staff #3 has not yet completed the required course which is overdue by 5 months.
During the stated hours of operation, there always shall be on the premises when one or more children are present one staff member who qualifies as program lead or director and an immediately available staff member, volunteer or other employee who is at least 16 years of age with direct means for communication between the two of them.
Staff #1 was observed alone supervising twenty-two children while the inspector was onsite. The center did not have another immediately available staff onsite.
Children under 10 years of age shall always be within actual sight and sound supervision of staff.
During the inspection, Staff #1 was observed leaving all children (ages 5-11) alone on several occasions to take departing children to parents.
The required ratio for children school-age to 9 years is one staff per eighteen children.
Staff #1 was observed alone supervising twenty-two children (youngest child 5 years old) during the inspection. Two staff would be required.
There was no menu posted.
Upon arrival of the inspector, Staff #1 was unsure of the number of children in attendance. No written attendance was available.
