Inspector Notes

A code compliance inspection was conducted on October 5, 2022. There were fourteen children present with two staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, and medication. A total of five staff records were reviewed. Information gathered during the inspection determined non-compliance(s) with applicable code sections and violations were documented on the violation notice issued to the program.

Evidence: The exemption from licensure letter was not posted. A copy was maintained in a file in a locked office.

Based on review, facility did not ensure there is a required out of state background check request by the end of the 30th day of beginning employment when an individual has lived outside the state of Virginia within the past 5 years.

Evidence: The record for staff #3 (start date July 27, 2022) had documentation of living outside the state of Virginia within the past 5 years. The record did not have a child abuse and neglect registry check within 30 days for the state the individual lived in within the past 5 years.
Plan of Correction: The child abuse and neglect registry search was completed and will be mailed.
