Inspector Notes

A monitoring inspection was initiated on 11/16/2022 and concluded on 11/16/2022. There were a total of 3 children (12 points) observed with 2 caregivers present. The children were engaged in the following: toy play, and songs. The home, 3 child records, 6 caregiver/household member records, evacuation drills, emergency supplies were inspected. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. Standard 8VAC20-800-100-A
Based on a review of children records, the Provider did not obtain documentation of a physical examination prior to a child's attendance or within 30 days after the first day of attendance.
Evidence: Child #3, first day of attendance 6/8/2022 does not have documentation of a physical examination.
Plan of Correction: The provider will ask the parents for a completed physical examination and file in the child's record.
Standard 8VAC20-800-180-A
Based on a review of caregiver and household records, the provider did not obtain for each caregiver and adult household member a current Report of Tuberculosis Screening form, every two years from the date of the first screening.
Evidence: The following caregivers were missing their updated TB screening
Caregiver #1 TB dated 9/11/2020
Household #1 TB dated 9/8/2020
Household #2 TB dated 9/23/2020
Plan of Correction: All TB screening will be updated.
Standard 8VAC20-800-60-B
Based on a review of children records, the provider did not obtain the parents written authorization for emergency medical care should an emergency occur.
Evidence: Child # 's record1, date of first attendance 8/9/2022, was missing the written authorization from the parents for emergency medical care.
Plan of Correction: The parent will complete the emergency authorization for care.
Standard 8VAC20-800-830-C
Based on a review of evacuation drills, the Provider has not conducted monthly fire evacuation drills.
Evidence: The Provider did not conduct a fire drill for the months of September and October 2022.
Plan of Correction: Fire drills will be conducted monthly and recorded on the form.
Standard 8VAC20-800-90-A
Based on a review of children records, the provider did not obtain documentation that the child has been adequately immunized before the child can attend the Family Day Home.
Evidence: The following children do not have a record of immunization record on file;
Child #2, date of first attendance 9/7/2021 (repeat violation)

Plan of Correction: The provider will ask the parents for a copy of the immunization records for the children.
