Standard 8VAC20-800-180-A
A tuberculosis (TB) screening is required every two years from the last screening.
An updated TB screening for Caregiver #2 was not completed and was overdue by four months.
An updated TB screening for household # 4 was not completed and was overdue by one month.
Plan of Correction: Do the screen test as soon as possible.
Standard 8VAC20-800-280
Poisonous materials shall be stored in an area inaccessible to children.
Poisonous cleaning chemicals were on a shelf in the bathroom accessible to children.
Plan of Correction: Put them away, never keep them out again.

Children's records are required to contain name and policy number of the child's medical insurance and documentation of review of the child's emergency information.
The record for Child #6 was missing documentation of review of emergency information, last review was completed 6/6/2024.
The record for child # 3 was missing the medical insurance policy number, the child had been in care for over 2 months.
Plan of Correction: Need to review and let parents signature on document and date once a year. Ask parents for policy number.
Standard 8VAC20-800-690-A
A child shall not be left unattended on a changing table during diapering.
Caregiver #1 left a two year old unattended on the changing table as she went to get a pair of gloves.
Plan of Correction: Keep gloves extra box near me, also put child down. Never do that mistake again.
Standard 8VAC20-800-830-A
The emergency evacuation procedures shall be practiced monthly with all caregivers and children in care during all shifts that children are in care.
The emergency fire drills were not practiced in November, the last documented drill conducted was on 10/16/2025.
Plan of Correction: Always remember to do one each month. We did one at 12:35 today with children.
Standard 8VAC20-800-960-E
Bottles shall be refrigerated and labeled with the child's full name and the date, if more than one infant is in care.
There were bottles for 4 infants that were found not dated.
Plan of Correction: Make sure to write the dates.
