Inspector Notes

An unannounced Code Compliance inspection was conducted with the Administrator on 2/9/2023 from 12:00pm through 1:30pm. There were 53 children present supervised by 9 staff. The children were observed playing with blocks, completing crafts, coloring, playing in centers, and helping clean up. Five child records were reviewed for documentation of immunizations and proof of identity and found to be in compliance. Five staff records were reviewed for background checks, annual health statements, and training. The program has multiple staff with current certification in CPR and First Aid. Areas of non-compliance are identified in the Violation Notice.

Based on staff interviews, the center has not established and implemented procedures for a daily simple health screening and exclusion of sick children by a person trained to perform such screenings.

Evidence: Staff advised that the center has a procedure to ensure a simple health check is conducted daily, however, no staff have received training in daily health observation.
Plan of Correction: Staff are currently being registered for Daily Health Observation training and will complete trainings as available.

Based on record review, the center did not ensure that all staff submit to a fingerprint-based criminal history background check prior to hire.

Evidence: Staff #4 (Date of Hire: 8/2/22) did not have the results of a fingerprint-based criminal history background check on file.
Plan of Correction: All fingerprint-based criminal history background checks have been completed and are on file.

Based on record review, the center did not obtain the results of a criminal history record search prior to hire or request a search of the child abuse and neglect central registry within 30 days of hire from all states in which staff have resided within the last 5 years.

Evidence:
1. Staff #4 (Date of Hire: 8/2/22) has resided in North Carolina within the last 5 years as documented on a Sworn Disclosure Statement signed by the staff on 1/23/23 The center did not have documentation requesting a search of the child abuse and neglect central registry from North Carolina for Staff #4.
2. Staff #5 (Date of Hire: 8/2/22) has resided in Delaware and Maryland within the last 5 years as documented on a Sworn Disclosure Statement signed by the staff on 1/23/23. The center did not obtain the results of a criminal history record search for Delaware and did not have documentation of requesting a search of the child abuse and neglect central registry from Maryland and Delaware for Staff #5.
Plan of Correction: We have contacted North Carolina to receive the proper forms to complete the background check.

We have received the record search from Delaware and are waiting for the documentation from Maryland.
