Standard 22.1-289.031-B-1
(Complaint Related) The center shall establish and implement procedures for handwashing by staff and children before eating and after toileting.

In a statement dated 11/19/25, Staff #1 wrote that she saw Child #1 (age 4 years) ?licking her fingers at her seat at lunch? on 11/18/25. In an interview on 12/04/25, Staff #2 stated Child #1 ?licked her hands multiple times throughout the day? on 11/18/25 and she noticed Child #1 ?had sanitizer in her hand and was licking it? after lunch. Staff #2 told Child #1 to stop and stated it looked like she had ?2 or 3 pumps? in her hand.

Child #1 was later taken to a local hospital after she was observed to be wobbly and unsteady. Medical records indicate that Child #1 had a blood alcohol level of .23 and had multiple episodes of vomiting. Child #1 was then air lifted to a second hospital for treatment.

In an interview on 12/04/25, Staff #1 reported that she did not know what the regular procedure was for hand washing. Staff #1 reported that sometimes children administer hand sanitizer to themselves under supervision and that sometimes a child will administer hand sanitizer to other children under supervision.

In an interview on 12/04/25, Staff #2 reported that she did not know if there were any policies regarding hand washing in writing. She reported that there isn?t any specific policy and that hand washing is left to the teacher?s discretion. Staff #2 stated that Child #1 ?gave herself hand sanitizer before going into the bathroom? before nap. Staff #2 said they supervise the children but acknowledged that they ?may not see everything?. She said that it?s ?not a set thing? whether the children wash their hands with soap and water rather than use hand sanitizer or disposable wipes.

In an interview on 12/04/25, Staff #3 reported that ?handwashing is class specific? and that ?teachers do whatever works for their classroom?
Plan of Correction: FCS has created a written policy which sets out a specific procedure by which hands must be washed, describes employee supervision requirements, and requires hands to be washed prior to consuming food, after toileting and diapering, and in other circumstances. In addition, FCS has made its child day center employees specifically aware of this new handwashing policy. (see attached)

(Complaint Related) The center shall establish and implement procedures for appropriate supervision of all children in care to ensure safety of children.

On 11/18/25, Child #1 (age 4 years) ingested hand sanitizer throughout the day resulting in her hospitalization and a blood alcohol level of .23. In an interview on 12/04/25, Staff #2 said they supervise the children when they administer hand sanitizer to themselves but acknowledged that they ?may not see everything?. In a phone interview on 12/08/25, Staff #4 stated that they don?t have supervision policies in writing but that all staff are taught that children are never to be unattended. Staff #4 said staff are taught to keep the bathroom door cracked when children are in the bathroom. Staff #1 reported that children ?go to the bathroom alone? and that they will ?check on them after 3 or 4 minutes or if they have an accident?. Staff #2 reported that Child #1 ?normally goes to the bathroom like 3 times a day? and ?the door is shut?.
Plan of Correction: FCS has created a written policy to be added lo its PreK handbook/operations manual which describes appropriate child supervision procedures for daily intake and dismissal of students, the supervision of students in various circumstances during the school day, and adherence to various student? teacher ratios throughout the school day. FCS has made its child day center employees specifically aware of these new policies (see attached).

(Complaint Related) The center shall establish and implement procedures for ensuring that all areas of the premises accessible to children are free of obvious injury hazards.

In an interview on 12/04/25, Staff #1 reported that one bottle of hand sanitizer was kept on a table by the classroom door and a second bottle was kept on a table outside the bathroom. Both were within reach of children. The manufacturer directions on the label of the hand sanitizer used by Child #1 on 11/18/25, has a warning to ?keep out of reach of children? and that ?children under 6 years should be supervised when using this product?. In a phone interview on 12/08/25, Staff #4 said that staff ?would be supervising use of Wet Ones wipes and hand sanitizer?. In an interview on 12/04/25, Staff #1 reported that sometimes one student is identified as a helper and that the ?helper? will walk around the lunch table to give each student some hand sanitizer. In an interview on 12/04/25, Staff #2 reported that sometimes children administer hand sanitizer to themselves. Staff #2 reported that one of the teachers is always in that section of the room to supervise the children, but she acknowledged that ?they may not see everything?. The label on the hand sanitizer used by Child #1 on 11/18/25 lists "Active Ingredient Ethyl Alcohol 70%".

In an interview on 12/04/25, Staff #1 reported that disposable hand ?wipes were in a drawer in the bathroom?. This was within reach of children. The manufacturer directions on the container of disposable hand wipes has a warning to ?keep out of reach of children?.
Plan of Correction: FCS has created a written policy to be added to its PreK handbook/operations manual and is designed to ensure that obvious injury hazards are inaccessible to children. The policy describes new storage requirements for cleaning supplies and other hazardous materials, as well as the types of cleaning supplies that may be used in the school and stored in classrooms. FCS has made its child day center employees specifically aware or this new policy (see attached).
