Inspector Notes

An announced code compliance inspection was conducted March 23, 2023. The licensing inspector was onsite from approximately 11:00am until 2:00pm. There were seven children present, ranging in ages from 14 months to four years, with three staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, and medication. A total of five children's records and nine staff records were reviewed.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Standard 22.1-289.031-A-6
Based on a review of the parent handbook and interview, the center failed to provide a written disclosure to parents and guardians of the children in the center and the general public that includes information regarding the enrollment capacity, health requirements of staff and public liability insurance.
Evidence:
1. During the inspection, the licensing inspector reviewed information regarding the center that is disclosed to parents or guardians in writing and made available to the general public.
2. The written information did not include enrollment capacity, health requirements and public liability insurance.
3. Staff #1 stated she was not aware this information needed to be included in the parent handbook.
Plan of Correction: The required information will be added to the Parent Handbook.
Standard 22.1-289.031-B-4
Based on the review of five children's records, the center did not establish and implement procedures to ensuring that two children in the center were in compliance with the provisions of ? 32.1-46 regarding the immunization of children against certain diseases.
Evidence:
1. The records for Child #2 (first date of attendance 08/01/2022) contained documentation of immunizations dated 02/14/2023.
2. The record for Child #4 (first date of attendance 06/21/2022) contained documentation of immunizations dated 06/28/2022.
3. Staff #1 confirmed this information during the inspection.
Plan of Correction: Going forward, we will ensure all documents, including immunization records are received prior to allowing children to attend the center.
