Inspector Notes

A code compliance inspection was initiated on Wednesday, April 3, 2024 and concluded on Wednesday, April 10, 2024. On April 4, the inspector was on site from 2:25pm to approximately 3:05pm. There were 15 children present with five staff members directly supervising. Medications are administered when needed and medications and authorizations were reviewed. A total of five children's records and five staff records were reviewed. Additional documentation was submitted electronically on April 9, 2024 and on April 10, 2024.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Based on a review of five staff records and observations, the center did not ensure one staff member in a supervisory position was certified by a practicing physician or physician assistant to be free from any disability which would prevent them from caring for children under their supervision.

Evidence: The staff health report in the record of Staff #1, employed on 04/17/23, was completed by a licensed practical nurse (LPN). During the inspection on 04/03/24, Staff #1 was observed supervising children.
Plan of Correction: A new staff health report will be obtained for Staff #1.
