Inspector Notes

This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, documents review, interviews.

A monitoring inspection was initiated on 9/22/2021 and concluded on 10/1/2021. The director was contacted by telephone and a virtual inspection was conducted. There were 29children present, ranging in ages from 6 months to 5 years, with 14 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of three child records and four staff records were reviewed.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Based on review of staff records the center failed to have all the required information in the staff files.
Evidence:
1. Staff # 1 (hire date 6/18/21) and staff #3 (hire date 6/15/21) did not have documentation of two or more references being contacted.
Plan of Correction: The director will ensure that all reference contacts will be documented.
