Inspector Notes
An unannounced Code Compliance inspection was conducted with the director and on May 25, 2023 beginning at 12:17pm and ending at 1:16pm. There were 13 children, ages 3 through 5 years old, in the care of 3 staff members during the inspection. The children were observed during nap time. Six children's records were reviewed and in compliance. Four staff records were reviewed. There were no medications on site to be reviewed. Facility policies and procedures were reviewed and in compliance.

Based on a review of documentation, the provider did not ensure that each person in a supervisory position has been certified to be free from any disability which would prevent them from caring for children under their supervision. Evidence: Staff A's record was missing documentation of a current health certification.
Plan of Correction: The facility will ensure that the staff has an updated health certification completed and documentation placed in their record.
