During the inspection, the inspector reviewed the areas listed above, to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.
Inspector Notes
An unannounced, on-site monitoring inspection was initiated and completed on December 18, 2024. The on-site inspection began at 11:30am and ended at 1pm. The inspector reviewed compliance in the areas listed above. There were 23 children present and 3 staff. The inspector reviewed 5
children?s records and 4 staff records on-site. This inspection document review, tour of the facility, interviews, and observations. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.
Standard 8VAC20-780-160-C
At least every two years from the date of the initial screening or testing, staff members shall obtain and submit the results of a follow-up tuberculosis screening. Staff #1 tuberculosis screening expired on May 3, 2024.
Plan of Correction: The center responded with the following: Staff #1 will obtain a tuberculosis screening this week and place the results in their file.
Standard 8VAC20-780-60-A
Each center shall maintain and keep at the center a separate record for each child enrolled which shall contain a work phone number of each parent who has custody. Child #1, Child #2, and Child #3 do not have work phone numbers listed of their parent who has custody in their record.
Plan of Correction: The center responded with the following: The center will obtain the work phone numbers for Child #1, Child #2, and Child #3 and place them in the child?s record.
Standard 8VAC20-780-70
For staff hired after March 1, 1996, the following staff records shall have documentation that two or more references as to character and reputation as well as competency were checked before employment. Staff #2 did not have two references in their file and has been employed for three months working with children.
