Inspector Notes

An unannounced, on-site monitoring inspection was initiated on 10/20/2025 and completed on 10/20/2025. The on-site inspection began at 10:00 am and ended at 12:50 pm. The inspector reviewed compliance in the areas listed above. There were 24 children present and 3 staff. The inspector reviewed 5 children's records and 3 staff records on-site on 10/20/2025.

Standard 22.1-289.035-A
All employees and applicants shall undergo a background check in accordance with subsection B prior to employment and every five years thereafter.
The fingerprint background check for staff 1 has not been updated every five years as required.
Plan of Correction: Staff 1 made an appointment on the date of the inspection for an updated background check.
Standard 8VAC20-780-160-C
At least every two years from the date of the first initial screening or testing, staff members shall obtain and submit the results of a follow-up tuberculosis screening as stated in subsection B of this section.
The TB screening for staff 2 was conducted on 06/14/2023.
Plan of Correction: The Director stated that an updated TB screening will be obtained for staff 2.
