Inspector Notes

An unannounced mandated inspection was completed from 2:55 p.m. until 4:25 p.m. There were10 children in care starting at 3:40 p.m. with one staff and three other staff arriving at 3:50 and after. Children were observed arriving at the program, having a snack, playing outside, and being picked up by their guardians.
Five children's records were reviewed. Three staff records were reviewed at the main office on 04/25/2022.
The inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued t the program.

Standard 8VAC20-780-245-J-3
Based on observation and interview, the center did not have staff who are MAT trained on site when a child was in care who is prescribed an epi-pen.
Evidence:
1. Staff #1 arrived at the after-school program at 3:40 and was not MAT trained. Child #1 who is prescribed an epipen was present.
Plan of Correction: The staff who is MAT trained was absent on this day. We are getting more staff MAT trained.
Standard 8VAC20-780-260-A
Based on record review, the center did not have an up to date annual fire inspection.
Evidence:
1. The last fire inspection was 10/29/2020.
Plan of Correction: The director was notified there is a vacancy with the the fire marshal. The fire chief has been notified but no one has called back.
Standard 8VAC20-780-340-I
Based on observation,each child was not greeted upon arrival at the center.
Evidence:
1. The inspector observed that center staff had not arrived in the cafeteria until 3:40 p.m. Children enrolled in the YMCA after-school program are supposed to be dismissed to the cafeteria at 3:05 p.m. Because no YMCA staff was present, children were kept in their classrooms at first and then in the cafeteria and supervised by school staff until YMCA staff showed up. School staff tried to call the main YMCA office to see if staff was on there way.
Plan of Correction: There was a staffing issue; in the future supervisors will verify with staff to make sure they know of any staffing coverage changes.

Based on observation, there was nonprescription medication not kept in the original container.
Evidence:
1. The inspector observed 5 pills labled diphenhydramine 12.5 mg. in a blister pack. They were not in the original container and not labeled.
Plan of Correction: The parent will be asked to bring in the original container.
