The licensing inspector initiated an unannounced remote inspection on 4/11/22 and concluded on 4/11/22 in response to a complaint received by the department on 3/10/22 for allegations regarding supervision and physical plant. Observations and interviews were conducted with the staff relating to the allegations. The information gathered during the investigation does not support the allegation, so the complaint is determined to be "not valid."
