Inspector Notes
The licensing inspector initiated an unannounced inspection on 06/12/2024 in response to a complaint received by the department on 05/29/24 for allegations regarding food service and supervision. Interviews, record, video review, and policy review were conducted relating to the allegations. The information gathered during the investigation supports the allegation in part, so the complaint is substantiated.

Based on a review of a video and interviews, the center failed to ensure areas and equipment of the center were kept in safe conditions which includes items that a child could pull down on themselves.
Evidence:
A highchair was placed on top of a table. A fifteen month old child pulled the highchair off of the table and it fell on him leaving red marks on his forehead and head.

Plan of Correction: All staff have been advised to not place highchairs on the table. They should be place in the hall if needed.
