The licensing inspector initiated an unannounced code compliance inspection on 06/12/2024 in response to a complaint received by the department on 05/29/24 for allegations regarding food service and supervision. Interviews, record, video review, and policy review were conducted relating to the allegations. The information gathered during the investigation supports the allegation in part, so the complaint is substantiated.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the facility.

Based on a review of the staff handbook and interview, the center staff failed to implement a procedure to ensure that all areas of the premises accessible to children are free of obvious injury hazards.

A highchair was placed on top of a table. A fifteen month old child pulled the highchair off
of the table and it fell on him leaving red marks on his forehead and head.
