Inspector Notes
An unannounced subsidy health and safety inspection (SHSI) was conducted in conjunction with the code compliance inspection on 10/17/2024 and completed on 10/17/2024 in response to a self-report received by the licensing office on 9/26/2024 relating to special feeding needs. Interviews were conducted on 9/25/2024 and 10/17/2024. This inspection included interviews.
The preponderance of evidence gathered during the investigation supports the allegation; therefore, the complaint is determined to be valid. Information gathered during the inspection determined non -compliance with applicable standards or law and the violations are documented on the violation notice issued to the program.
Violations
1A one-year-old child was placed in a high chair and the protective belt was not securely fastened as the straps were not tightened to secure the child. The child fell out of the highchair and hit their head on the floor, which caused a bump on the child's head.
