Inspector Notes

An unannounced, on-site renewal inspection was initiated and completed on 7/25/2024. The on-site inspection began at 10:10am and ended 11:00am. The inspector reviewed compliance in the areas listed above. There were 3 children present and 1 caregiver; the point total was 8. The inspector reviewed 3 children?s records, 1 caregiver record, and 2 household member records on-site. This inspection included document review, tour of the facility, interviews, and observations. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Based on record review, the provider did not review the emergency plan atleast annually.
