Areas and equipment of the center, inside and outside, shall be maintained in a safe and operable condition. The following physical plant safety and maintenance issues were observed. Peeling paint in multiple areas of the classrooms. The light switch covers in two of the classrooms were off and exposing electrical wires.

Each center shall maintain and keep at the center a separate record for each child enrolled which shall contain all required information. The record for child #1 did not have documentation of the work contact information for one parent.
