Inspector Notes
An unannounced inspection was conducted today. There were 6 children present with 2 caregivers supervising. The children were observed playing with floor toys and singing songs. They were also observed hand washing an eating lunch. Lunch consisted of cauliflower, rice, oranges and milk. The physical plant, attendance logs, evacuation logs, first aid kits, menus, children records, caregiver records and household member records were reviewed. Child #1's record did not have documentation of a doctor's address, authorization of emergency pick or a medical insurance policy number.
Child #2 record did not include documentation of one of the parents work address or phone number or the doctors phone number.
Child #3's record did not include documentation of a parents work address or phone number, the other parent's work number, the doctor's phone number or the medical insurance policy number.
