Inspector Notes

An unannounced renewal inspection was conducted on April 3, 2023. There were 5 children (12 points) with 2 caregivers. The home, children's records, caregiver records, household member records, pet records, first aid kits, menu's and cribs were reviewed. Areas of non-compliance were cited on the violation notice. Standard 8VAC20-800-100-A
Based on a review of children's records, the provider failed to obtain documentation of a physical examination prior to a child's attendance or within 30 days after the first day of attendance.

Evidence:
Child #3's record does not contain documentation of a physical exam by the 30th day of attendance. First Day of Attendance - 08/02/2021.
Plan of Correction: I will request the physical from parent.
Standard 8VAC20-800-320-B
Based on observation, the provider failed to ensure that child-resistant protective covers larger than 1 1/4 inches in diameter shall be installed on all unused electrical outlets and surge protectors accessible to children under five years of age.

Evidence:
Outlet cover was missing from outlet in the den.
Plan of Correction: Outlet cover was replace during inspection.

Based on a review of pet vaccinations, the provider failed to obtain/maintain documentation of the current rabies vaccinations.

Evidence:
Pet #2's records did not have documentation of current rabies vaccination.
Plan of Correction: I will request documentation from vet.
