An unannounced inspection was conducted today.
There were 23 children present with two staff. The children were eating a snack and playing games.
Four child records were reviewed. Two staff records will be reviewed at the main office.

Based on a review of staff files, the facility failed to ensure that all staff have a fingerprint criminal record check prior to the first day of work.
Evidence:
1. Staff #1 was hired on 5/27/22. The fingerprint background check is dated 7/11/22.
2. Staff #2 was hired on 7/11/22. The fingerprint background check is date 8/1/22.
Evidence:
1. Staff #1 was hired on 5/27/22. The fingerprint background check is dated 7/11/22.

Based on observation, the facility failed to ensure that children under 10 years of age are always within sight and sound supervision of staff.
Evidence:
1. There are two classrooms which are across the hall from each other. There are children in both classrooms. For 10 minutes while staff #1 was at the entrance of the building with a new client, staff #2 was driving the van and staff #3 was moving between the two classrooms. There were children under 10 years of age in both classrooms.

Based on observation, the facility failed to ensure that children's hands are washed with soap and water or using disposable wipes before and after each meal or snack; And that staff wash their hands before preparing food.
Evidence:
1. Children did not wash their hands before or after eating the afternoon snack.
2. Staff did not wash their hands before distributing a snack.
