Inspector Notes

An unannounced, on-site renewal inspection was completed on 05/08/2025, as a part of the licensure period. The on-site inspection began at 1:45 pm and ended at 2:30 pm. The inspector reviewed compliance in the areas listed above. There were 17 children present and 3 staff. The inspector reviewed 5 children?s records and 5 staff records. This inspection included a document review, tour of facility, interviews, observations and measurements. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Standard 8VAC20-780-130-E
The center shall obtain documentation of additional immunizations once every six months for children under the age of two years.

The record for Child #2 did not have updated immunization documentation, this was overdue by approximately two months.
Plan of Correction: Center requested information from parent.
Standard 8VAC20-780-60-A
Each center shall maintain and keep at the center a separate record for each child enrolled that contains all of the required information.

The record for Child #1, enrolled for seven months, was missing the following required components: parent's work phone numbers and addresses for emergency contacts.

The record for Child #4, was missing the following required components: father's home phone number, parent's employment information, emergency contacts, and first date of attendance.

The record for Child #5, was missing the following required components: father's home address, addresses for emergency contacts, and first date of attendance.
