Inspector Notes

An unannounced complaint investigation was initiated on 06/06/22 and concluded on 06/29/22 in response to a complaint received by the licensing office on 06/06/22 regarding supervision and procedures for emergencies. Observation, interviews, and record reviews were conducted related the allegations. The information gathered supported the allegations in the area of procedures for emergencies making the complaint valid. Areas of noncompliance are identified on the violation notice and were discussed with the owner and director at the conclusion of this investigation.

Based on record review and interview it was determined the center did not maintain a written record of children's serious and minor injuries that contained all of the required information.
Evidence:
1. The injury report for Child #1, #2, and #2 dated 6/3/22 did not contain the name of parent, time, method of contact, and future action to prevent recurrence.
2. The injury report for Child #1 only contained one of the two required signatures.
Plan of Correction: Owner and Director will retrain staff on completing injury reports.
