Inspector Notes
An unannounced monitoring visit and facility tour was conducted on 9/20/2022. A self report by the center on 9/13/22 and a subsequent complaint filed online on 9/15/22 for an injury was also investigated during this inspection. At the time of inspectors' arrival there were 42 infant to preschool aged children in care with 7 staff members and 2 administrators. Children were observed interacting with staff, nap, lunch and hand washing. A sample of 5 children's records and 5 staff records were reviewed. Injury reports were also reviewed. Areas of noncompliance are identified on the violation notice and were discussed with the director in an exit meeting at the conclusion of this inspection.

1. The record for staff #5 did not contain documentation of an updated TB screening. The documentation available was dated 3/17/18.

Evidence:
1. On 9/12/22 around 3:00 p.m. child #1(16 month old) fell out of a 14" chair which is designated for use by older preschool to school aged children.
2. Child #1 sustained a small lump on the right side of her face with slight bruising that later scabbed.
3. Staff #2 reported seeing child #1 fall from the chair onto the carpet.
4. Staff #4 reported hearing child #1 cry and looked up to see her on her knees on the carpet in front of the chair.
5. Staff confirmed that the chair in the classroom was 14" in height.
6. Staff #1, staff #2, and staff #4 from the infant classroom did not notice the injury until it was pointed out by staff #6 and #7 from another class inquired about the mark on the child's face around 5:00 p.m. It was around this time that the parent picked up the child from care.
