Inspector Notes

An unannounced, on-site renewal inspection was completed on 07/08/2025, as a part of the licensure period. The on-site inspection began at 1:00 pm and ended at 2:30 pm. The inspector reviewed compliance in the areas listed above. There were 33 children present and 7 staff. The inspector reviewed 5 children?s records and 5 staff records. This inspection included a document review, tour of facility, interviews, observations and measurements. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Standard 8VAC20-780-130-E
The center shall obtain documentation of additional immunizations once every six months for children under the age of two years.

The record for child #4 did not have updated immunization documentation, it was overdue by five months.
Plan of Correction: Center will request from parent.
Standard 8VAC20-780-270-A
Areas and equipment of the center, inside and outside, shall be maintained in a clean, safe and operable condition.

There were loose cords, accessible to children in care, hanging from receptacle level of the wall by the front entrance.
Plan of Correction: Cords will be secured.

Each center shall maintain and keep at the center a separate record for each child enrolled which shall contain the required information.
Evidence:
The record for child #4 did not contain the parent's employment information.
