Inspector Notes

An unannounced, on-site monitoring inspection was initiated and completed on September 10, 2024. The on-site inspection began at 10:00am and ended at 11:15am. The inspector reviewed compliance in the areas listed above. There were 8 children present and 3 staff. The inspector reviewed 3 children's records and 3 staff records on-site. The inspection included document review, tour of the facility , and observations.Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Based on review of staff records, there is no documentation of staff completing Daily Health observation training.

Evidence
There were no staff on site who completed Daily Health observation training during the inspection. No staff have completed a daily health training observation training.
Plan of Correction: Staff will complete the DHO training and send the certificate to the inspector.

Based on review of children's records, staff did not request at least annually parent confirmation that the required information in the child's record is up to date.

Evidence
The record for child 2 not have an annual parental review of the child's record. Last review of the record was 10/18/2021.
Plan of Correction: We will make sure to have the parents review the child's record annually.

Based on review of the written record of children's serious and minor injuries did not contain all the required information.

Evidence
The documentation of children's serious and minor injuries did not include any future action to prevent recurrence of the injury;
