During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. Inspector Notes

An unannounced, on-site monitoring inspection was initiated and completed on 6/5/2025. The on-site inspection began at 12:00pm and ended at 1:25pm. The inspector reviewed compliance in the areas listed above. There were 14 children present and 4 staff. The inspector reviewed 5 children?s records and 4 staff records on-site on 6/5/2025. This inspection included: document review, tour of the facility, interviews, and observations. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Standard 8VAC20-780-270-A
Areas and equipment of the center, inside and outside, are required to be maintained in a clean, safe and operable condition.

A corner of the wall in the movement matters room was broken and the blue drywall was breaking off.
Plan of Correction: We will put a ticket in today to get the drywall fixed and will look into adding bumpers for the wall corners.
Standard 8VAC20-780-500-B
The diapering area is required to have a nonabsorbent surface for diapering or changing. For children younger than three years, this surface shall be a changing table or countertop
designated for changing.

The changing pad used for diapering in the Infant room had a rip in it and was no longer nonabsorbent.
Plan of Correction: We will replace the changing pad today with a new one.

Prepared infant formula is required to be refrigerated, dated and labeled with the child's name.

There were 3 bottles in the infant room refrigerator that were not labeled with the date for the day.
Plan of Correction: We will remind parents to date the bottles and have the teachers check the bottles daily.
