Inspector Notes

An in-person mandated monitoring inspection was conducted on December 9, 2022 from 10:30am until 11:50pm. There were 58 children present with 8 staff supervising. Children were observed playing outside freely, playing in centers, working independently, and preparing for lunch. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 5 child records and 5 staff records were reviewed.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Based on record review, the center did not ensure all out of state background searches were completed for the states in which staff resides in the preceding 5 years.

Evidence: Staff 1 does not have completed out of state searches. (Staff 1, 9/13/2022)

Based on observation, caregivers did not wash hands in between diaper changes.

Evidence: Caregiver 1 was observed not washing hands in between diaper changes.
Plan of Correction: Administration discussed handwashing procedures with staff.
