Inspector Notes

An unannounced monitoring inspection was conducted today between 12:10pm-1:00pm. There were 5 children present with the Provider for a total of 15 points within the supervision guidelines. The children were observed taking a nap and being held. Positive interactions were observed during this inspection. A complete inspection of the physical plant, children and staff records, evacuation drill log, attendance and emergency materials were observed during this inspection. Violations were found during today?s inspection and can be viewed on the Violation Notice. Thank you for your cooperation. Standard 8VAC20-800-540-A
Based on observation, infant seats were not used according to the manufacturer's instructions and when occupied by a child.

Evidence:
1. Two infants were observed sleeping in bouncy chairs. The bouncy chairs are not made to be used for sleeping purposes according to the manufacturer's instructions.
Plan of Correction: I will make sure the infants sleep in their cribs.
Standard 8VAC20-800-90-C
Based on a review of children's records, the family day home did not obtain documentation of additional immunizations for 3 out of 5 children.

Evidence:
1. Child #3's (dob: 4/10/2021) last immunizations on file at the time of the inspection were dated 4/19/2022.
2. Child #1's (dob: 5/21/2021) last immunizations on file at the time of the inspection were dated 8/26/2022.
3. Child #5's (dob: 1/29/2022) last immunizations on file at the time of the inspection were dated 7/29/2022.
Plan of Correction: I will ask all of the parents to bring the updated immunizations for all of the children.
