Inspector Notes
An unannounced, on-site monitoring inspection was initiated and completed on 1/31/2025, as a part of Intensive Plan of Correction (IPOC) follow-up. The on-site inspection began at 2:30pm and ended at 3:10pm. The inspector reviewed compliance in the areas listed above. There were 0 children present and 1 caregiver; the point total was 0. The inspector reviewed 2 children?s records and 6 caregiver/household members records on-site on 1/31/2025. This inspection included: document review, tour of the facility, interviews, and observations. Information
gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.
Violations
1The Provider and 3 household members all had Tuberculosis screenings that have expired.
