Inspector Notes

An unannounced, on-site monitoring inspection was initiated on 5/09/2024 and the inspection concluded on 5/09/2024. The on-site inspection began at time 4:30 p.m. and ended at 5:50 p.m. The inspector reviewed compliance in the areas listed above. There were 51 children present and 8 staff. The inspector reviewed 5 children?s records on site and 3 staff records electronically on 5/16/2024. This inspection included document review, tour of the facility, interviews, and observations.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Standard 8VAC20-780-240-A
Staff shall complete the Virginia Department of Education-sponsored orientation course (VA Pre-service) within 90 days of employment. Staff 2 (date of employment 10/2/2023) did not have documentation of having completed the VA Dept of Education sponsored orientation course within 90 days of employment.
Plan of Correction: All staff will complete the 10 hour VA Preservice training within 90 days of employment.
Standard 8VAC20-780-70
The facility failed to have required documentation in each staff file. Two references (not relatives by blood or marriage) as to character, reputation, and competence should be checked prior to employment. Evidence: Staff 1 (date of employment 1/29/2024) had a reference related by marriage.
Plan of Correction: Management will remind HR that references can not be related.
