During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced, on-site renewal inspection was initiated on April 25, 2025 and the inspection concluded on May 7, 2025. The on-site inspection began at 3:15 p.m. and ended at 5:15 p.m. The inspector reviewed compliance in the areas listed above. There were 33 children present and 7 staff. The inspector reviewed 5 children?s records on site and 2 staff records electronically on May 1. 2025. This inspection included document review, tour of the facility, interviews, and observations.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Standard 8VAC20-780-510-E
The center shall ensure that procedures for administering medication shall include methods to prevent use of outdated medication. The emergency medication provided for one child 2/2025 had expired 12/2023 and the parent had not been notified that a replacement was needed.
Plan of Correction: The medication will be returned to the parent and a current medication will be requested.
Standard 8VAC20-780-60-A
Repeat violation:
Each center shall maintain and keep at the center a separate record for each child enrolled which contains required information. The center did not have access to the children's physical and immunization records that are housed in the school office. .

A written care plan is required for each child with a diagnosed food allergy, to include instructions from a physician regarding the food to which the child is allergic and the steps to be taken in the event of a suspected or confirmed allergic reaction. Child 2 who has been enrolled for 7 months, has a diagnosed food allergy and did not have a written care plan in their file.
Plan of Correction: The new regional director will assist in completing children's files.
