Inspector Notes

This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.

A code compliance inspection was conducted on May 13, 2021. The director was contacted by telephone to initiate the inspection. There were 7 children present and two staff. The inspector emailed the director a list of items required to complete the inspection. The Inspector reviewed two staff records submitted by the facility to ensure documentation was complete.

Information gathered during the inspection determined non-compliance(s) with applicable code sections and violations were documented on the violation notice issued to the facility.

Based on record review, in one of two staff records reviewed, the center did not obtain a copy of the results of a sex offender registry check and a search of the child abuse and neglect registry or equivalent registry from any state in which the individual has resided in the preceding five years.

Evidence: Staff #1 indicated as to living outside of the state of Virginia within the past five years on the sworn statement. The director confirmed that there was no documentation of results of an out of state sex offender registry check or results of an out of state central registry finding, or its equivalent.
Plan of Correction: The Director obtained the results of the sex offender registry check and forwarded the results to the licensing inspector. The center will have the staff person to complete the documentation for the out of state central registry and results will be forwarded to the licensing inspector once they are returned.
