Inspector Notes
A subsidy health and safety inspection (SHSI) was conducted on May 1, 2023. There were thirty children present with five staff supervising. A code compliance inspection was also conducted on this date. Information gathered during the inspection determined non-compliance(s) with applicable standards or law, and violations were documented on the violation notice issued to the program.

Based on observation, facility did not ensure areas and equipment of center, inside and outside, are maintained in a clean, safe and operable condition. Unsafe conditions shall include items that a child could pull down on himself.
Evidence: A night light and tablet were stored on a shelf on the wall by a crib. The cords to these items were within reach of an infant observed napping in the crib.

Plan of Correction: The items will be removed from that shelf and plugged into outlets that are not within reach of children.

Based on observation, vendor did not ensure children less than 10 years of age are always within actual sight and sound supervision of staff, except that staff need only be able to hear a child who is using the restroom.
Evidence: At approximately 12:50, child #1 was observed in a crib in an area outside the toddler room and there were no staff present. Staff stated the child had a temperature and was waiting to be picked up.
