8VAC20-790 Subsidy Program Vendor Requirements for Child Day Centers

Inspector Notes

A subsidy health and safety inspection (SHSI) was conducted on April 22, 2024 from approximately 1:20 pm to 2:40 pm. There were thirty-four children present with seven staff supervising. A code compliance inspection was also conducted on this date. Both inspections combined cover subsidy health and safety regulations. Only subsidy requirements that are not covered in code compliance inspection were reviewed during this supplemental inspection. Violations found are noted on the violation notice.

Based on review, facility did not ensure 1 of 4 child records have documentation of required information.

Evidence: The record for child #1 (start date 7/16/2023) did not have documentation of immunization record and proof of child's identity/age.
Plan of Correction: Staff will get the information from parent.
Standard 8VAC20-790-730-G
Based on observation, facility did not ensure there is at least 12" of space between occupied cribs and cots.

Evidence: Children were observed during naptime in four areas. Occupied cribs and cots were observed within a range of 4" - 6" between several sleeping children.
Plan of Correction: Staff will remove unused cribs from the infant room to allow space between cribs. Cots will be placed 12" apart during naptime.
