During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced, on-site monitoring inspection was initiated on and completed on 9/25/2025. The on-site inspection began at 11:25am and ended at 12:35pm. The inspector reviewed compliance in the areas listed above. There were seven children present and two caregivers; the point total was sixteen. The inspector reviewed five children?s records and three caregiver/household members records on-site. This inspection included document review, tour of the facility, interviews, and observations. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Standard 8VAC20-800-180-A
A tuberculosis (TB) screening is required every two years from the last screening. An updated TB screening for Caregiver #1 was not completed and was overdue by three months.
Plan of Correction: The provider will have their TB screening updated.
Standard 8VAC20-800-240-A
'Areas and furnishings of the family day home, inside and outside, shall be
maintained in a clean, safe, and operable condition. The children's play area had peeling paint on the baseboards.
Plan of Correction: The provider will have the baseboards repainted.
Standard 8VAC20-800-90-C
Documentation of additional immunizations is required once every six months for children under the age of two years. 'Child #3, two years and four months old, last documented immunization was over 12 months ago.
Plan of Correction: The provider will obtain an updated immunization record from the parents of the child.
