Inspector Notes
This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.
A non-mandated focused inspection was conducted on 3/24/2021. The provider was contacted by telephone to initiate the inspection. There were no children present with the provider. The inspector emailed the provider a list of items required to complete the inspection. The Inspector reviewed 1 new adult household member's record submitted by the facility to ensure documentation was complete. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the facility.
The Licensing Inspector has reviewed with the provider COVID-19 Essential Guidance for Child Care programs.
Please complete the columns for "Plan of Correction" and "Date to be Corrected" for each violation cited on the Violation Notice, and then return a signed and dated copy to the licensing office by 4pm on Wednesday 3/31/21. Please email me at morgan.bryson1@dss.virginia.gov with any questions.
